  • List A - Accepted evidence is: One of the below

    • a signed CMR document
    • a bill of lading
    • an airfreight invoice
    • Sea waybills

     

    List B – Second piece of evidence. This must be issued by a second party to the first piece of evidence.

    • an insurance policy regarding the dispatch or transport of the goods,
    • an invoice from the carrier of the goods (only if the document from List A is issued by a different company to this document)
    • bank documents evidencing payment for the dispatch or transport of the goods
    • official documents issued by a public authority, such as a notary, confirming the arrival of the goods in the Member State of destination (if applicable)
    • a receipt issued by the customer/warehouse keeper in the EU Member State of arrival, confirming the storage of the goods in that Member State

      

    Written Statement - The written statement must state:

    • the date of issue
    • the name and address of the acquirer
    • the quantity and nature of the goods
    • the date and place of the arrival of the goods
    • the identification of the individual accepting the goods at the time of collection on behalf of the customer
